Position Purpose: This position is responsible to supervise, coordinate and provide clinical leadership for the care coordination services provided by teams of care coordinators to adults with Special Needs Basic Care services.

Main Duties and Responsibilities:
- Supervise a team of care coordinators to ensure the effective delivery of services. Monitor team to ensure services to clients are meeting agency expectations and all documentation is in accordance with MHR documentation standards and State and Federal Mandates.
- Provide ongoing coaching, mentoring and supervision in an atmosphere for open communication to ensure case managers are supported in their roles and understand performance expectations. Work with Clinical lead to facilitate team meetings to provide case consultation to care coordinators regularly. Review and evaluate the performance of care coordinators in accordance to MHR policies and procedures.
- Collaborate with Human Resources and complies with MHR policies and procedures with staff recruitment, orientation, and training of care coordinators. Maintain program coverage through hiring, managing PTO requests and coordinating work schedules in the program.
- Attend all scheduled health plan meetings, manager meetings and trainings. Collaborate with the health plan to facilitate open communication and resolve issues and concerns as they arise.
- Complete CEU requirements, IAW Rule 79, County and MHR requirements.
Minimum Education and Experience
- Four-year degree required, and least two years of experience in case management or care coordination preferred.
- Must be a Licensed Social Worker by the State of Minnesota.
- Licensed as a mental health professional in the State of Minnesota and Master’s degree preferred.
