@Home Therapy is a rehabilitation service provider of PT, OT, and SLP services for adults living at home. We are seeking a licensed Speech Therapist who can travel to patients’ homes for Evaluations and Treatments. This Speech Therapist position is Per-Diem, enabling you the flexibility to accept patients in your local area and decline or accept patients according to your schedule. @Home Therapy provides extensive on-boarding for new therapists as well as ongoing therapist support for clinical or documentation questions.In addition to per-diem reimbursement, we are now offering a sign-on bonus for qualified therapists.

We seek Speech Therapists with the following qualifications:
- Valid Speech Therapist License and Registration in New York required.
- Experience in Geriatric Rehabilitation required.
- Great Communication Skills are required.
- Familiarity with EMR software is a plus.
- Homecare Experience is a plus.
Responsibilities of the Speech Therapist include:
- Communicate with the office staff regarding your patients.
- Schedule your visits directly with your patients.
- Evaluate and develop a plan of care according to the patients' prior and current functionality.
- Provide follow-up visits in accordance with your plan of care and the patient's progress towards goals.
- Document your visits in the web-based EMR in accordance with Medicare's guidelines.
- Maintain active credentials and perform all visits within HIPAA and company compliance.
