Job Description
Extended Families of Nebraska, LLC provides day and residential services to persons with intellectual and/or developmental disabilities (I/DD). An Shared Living Provider supports individuals with developmental disabilities within their home. Candidates must be determined to provide person-centered supports to meet the needs of the people they serve throughout their lives. This includes creating unique supports to meet each person's needs and encouraging them to live a fulfilling life through independence and self-advocacy. They must be equipped and willing to honestly and ethically support people with respect and dignity.
The SLP process begins with applying and interviewing, followed by an environmental survey and visits with possible referrals. Placement is not guaranteed and may take an extended amount of time. Completing training will be an ongoing expectation.
Duties include:
- Providing services in a manner that respects the dignity and rights of the individual, protects the rights of the individual, and ensures that the individual is free from abuse, neglect and exploitation
- Ensuring that all services stated in the contract with EFN and ISP are provided according to the needs of the individual
- Ensuring all medical needs are being met in cooperation with residential entity, including scheduling, transporting, communicating with medical professionals, and ensuring all orders are being followed accurately and timely
- Maintaining a clean, safe and suitable environment related to the needs of the person supported
- Teaching and encouraging well-balanced meals, especially regarding modified diets
- Providing safe and reliable transportation that meets the individual’s needs
Must be able to pass the following background checks:
- Nebraska Department of Health and Human Services Adult Protective Services Central Registry
- Nebraska Department of Health and Human Services Child Protective Services Central Registry
- Essential Screening Backgrounds (private consulting firm)
Successful candidates must be highly organized, a self-starter, can multi-task, as well as have intermediate level computer skills. Knowledge of “Therap” is highly recommended. Two years of experience in direct support field of developmental/intellectual disabilities is expected. Applicants must be at least 21 years of age, possess a valid driver’s license, automobile insurance with a reliable vehicle, and home/renter’s insurance. Candidates must have a working computer, printer, and scanner.
