Responsibilities
GENERAL PURPOSE:
The Team Lead provides oversight to a team of Children's Health Home (CHH) Care Managers as part of the GCH@S, Health Home Program. The Team Lead will provide training, supervision and support to a team of eight CHH care managers in order to assure that comprehensive, care management services are provided to enrolled patients in accordance with established NYSDOH Children's Health Home requirements, GCH@S CHH protocols, Hospital and Social Work Division policies and procedures. The Senior Social Work Assistant/Team Lead assists the CHH Senior Social Worker in facilitating the integration of the Children's Health Home program services with general pediatric practices, sub-specialty practices, and child and adolescent behavioral health practices. The team lead role provides coverage for the Senior Social Worker.
SPECIFIC RESPONSIBILITIES:
- In collaboration with the CHH Senior Social Worker responsible for recruitment, onboarding, continuous training and supervision of 8 CHH care managers. Provides weekly supervision and/or group supervision promoting reflective practices, cultural sensitivity, patient centered care and ongoing professional development. Supervision also includes focus on efficient delivery of timely, appropriate and effective services, productivity and compliance with required standards. Assures that all mandatory trainings including certification in the CANS-NY occurs. Completes competency assessments and performance evaluations with input from members of the care team. Applies personnel management action plans as needed. Monitors caseloads for acuity, discharges and CHHUNY expectations Works with Quality and Compliance to assure staff are meeting audit requirements as regulated by NYS and CHHUNY.
- Assists in the identification and management of possible child maltreatment, IPV, acute psychiatric event or other adverse event and flows Hospital, SWD and CHH policies for reporting and management.
- Participates in NYS and CHHUNY trainings and meetings and disseminates information to staff. Facilitates coordination of CHH activities with GCH@S Social Workers, discharge coordinators and care coordinators.
- Provides coverage services as requested during the absence of co-workers.
- Completes CHH, Hospital and Social Work Division required training within the required time frames. Participates in ongoing professional development.
REQUIREMENTS:
- The Team Lead must have a bachelor's degree in Social Work, psychology or other human service degree and 3-5 years of relevant experience required
- E xperience preferably with a pediatric population in either a medical, behavioral health or child welfare setting and/ or in health home care management. Supervisory experience preferred.
- The candidate must demonstrate strong critical thinking, clinical assessment and service planning skills, strong oral and written communication skills, ability to engage diverse populations, comfort in completing home visits, skill in working as part of an interdisciplinary team, knowledge of community resources relevant to the target population. Further, the candidate must demonstrate proficiency in Microsoft Office including Word and Excel and electronic documentation systems required.

Pay Range
Pay Range: $49,795 - $69,701 Annually
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
