Position Responsibilities:
- Provide leadership during assigned shifts.
- Plan and organize activities with input from the individuals on a weekly; monthly and seasonal basis with community inclusion.
- Assist Program Manager with scheduling and attending medical appointments.
- Assists residents with ADL's (which may include the use of adaptive equipment); dining guidelines / protocols.
- Help prepare meals, clean, do laundry, grocery shopping, and help keep the home pleasant and comfortable.
- Transportation to/from appointments, community events, etc.
- Complete daily shift notes, data tracking, and any other individual record keeping that may be assigned.
- Assist with the development and implementation of individual support goals.
- Administer medications once certified.
- Remain current on agency certifications / trainings.
- Other programmatic tasks assigned by the Program Director / Site Manager.
Qualifications:
- High School Diploma or GED is required; college degree preferred.
- Valid drivers license for a minimum of 6 months; and acceptable motor vehicle record.
- Experience working with DB / ABI population; minimum 1yr experience preferred.
- Physical ability to perform the requirements of this position and ensure residents safety.
- Basic computer/office skills.
- Background check (CORI).

- $17.53/hour $1.00 wage differential for Medication Administration Program (MAP) certification.

- Saturday: 7am-11pm
- Sunday: 3pm-11pm
