Our center specializes in nursing care and short term rehabilitative services to aid in transitioning our community members back home. Sharon Care Center has been the centerpiece of post acute care for our partner hospitals and carries with it a team of highly regarded physicians that contribute to the implementation of its care model.
Our team is looking to add a Full-Time Social Services Director to our team.

Enrich our patients/residents' lives and your own as a Senior Social Services Director where you will serve as a Social Services expert and resource for social work practice within an assigned territory and the Social Services leader in a nursing center.
Position Highlights
*Report directly to the Nursing Home Administrator.
*Serve as a member of the interdisciplinary and management team of the nursing center.
*Plan, develop, organize, evaluate and direct the overall operation of the Social Services department in accordance with the National Association of Social Workers (NASW) Code of Ethics while maintaining compliance with regulations, policies, and federal/state/local
*Foster a climate, policies, and routines that enables patients/residents to maximize their individuality, independence, and dignity and the highest practical level of physical, mental, and psychosocial well-being and quality of life.
*Lead, guide and direct the Social Services staff.
*Educate all nursing center staff about the role of Social Services and the psychosocial needs of patients/residents and their families/significant others.
*Identify and monitor community changes and opportunities such as legislation, regulations, and programs that impact nursing home patients/residents.

Restrictions apply based on collective bargaining agreements, applicable state law and factors such as pay classification (full-time, part-time, or casual), job grade, location, and length of service.
Qualifications
*Bachelor's degree in Social Work from an accredited school of Social Work is required. Master's degree preferred. *Must possess any certifications/licensures as required by State of employment to practice in long term care. *Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Abuse preferred. *At least 3-5 years of supervised social service experience in a health care setting working directly with individuals preferred. *Management/administrative/supervisory experience is preferred.
