JOB SUMMARY: The Senior Care Coordinator will work across the Certified Community Behavioral Health Clinics, Pharmacy Services, and Health Home Programs
DUTIES AND RESPONSIBILITIES:
Health Homes
- Conducts field/home visits to clients in the Health Home Program.
- Reviews intake packages, including signed Health Home consent Forms gathering demographic information from clients.
- Completes behavioral risk assessment/reassessment for the collection of information to determine a person’s eligibility for Health Home Care Coordination Services
- Develops action plan with clients to reduce barriers and increase access to needed services
- Conduct fieldwork in all 5 boroughs.
Pharmacy Services
- Contact clients to see if they are interested in enrolling in BAC’s Pharmacy Services Program
- Complete forms to ensure the client’s enrollment in the Pharmacy Services program
- Educate staff members about the Pharmacy Services program
KNOWLEDGE, SKILLS AND ABILITIES:
- Knowledge in trauma-informed recovery, both in substance use disorders and mental health.
- Good interpersonal communications skills and the ability to empathize, relate, and effectively work with participants with chronic illness, substance use, and mental illness, or a history thereof.
- Experienced with working with clients with diverse backgrounds, cultures, races, sexual orientations and gender identities or expressions.
- Knowledge of community resources for individuals who have mental or substance abuse concerns.
- Possesses excellent judgment and the ability to maintain professional relationships and boundaries when working with clients.
- Demonstrates ability and willingness to utilize public transportation to navigate the five boroughs of NYC.
- Experience with and knowledge of Community Oriented Recovery and Empowerment program and Health and Recovery Plan preferred but not required.
EDUCATION:
- MINIMUM: Bachelor’s degree in any field with five (5) years of experience working directly with persons with behavioral health diagnoses.
- A master’s degree in one of the qualifying education fields may be substituted for one year of experience.
WORK EXPERIENCES:
- In providing direct services to people with Serious Mental Illness, developmental disabilities, or substance use disorders; OR
- In linking individuals with Serious Mental Illness, developmental disabilities, or substance use disorders to a broad range of services essential to successful living in a community setting (e.g. medical, psychiatric, social, employment, educational, legal, housing and financial services).
