Job Description Summary
Responsible for intake, scheduling, performing data entry and providing customer service to patients, family members/caregivers, and providers.
Essential Job Functions/Responsibilities
- Demonstrates professional conduct and ethics according to the agency policy and procedure.
- Maintains confidentiality of patient information and all agency operations.
- Performs clerical functions such as obtaining and/or completing appropriate paperwork, filing, copying, scanning, and tracking.
- Intake, gathering new patient paperwork, consents, medical history, entering new referrals into EMR and verifying insurance coverage.
- Schedules patient visits according to company policy and procedures for providers.
- Data input of schedules and client information into company EMR system daily.
- Confirms patient visits and completes chart preparation.
- Oversees caseload count daily for each provider.
- Collaborates daily with providers on schedules and issues.
- Ensures effective communication and positive working relationships with personnel from all areas of the company to maintain coordination of patient visits.
- Responsible for day-to-day staffing of patients to include scheduling reoccurring monthly, quarterly, and annual visit types according to CMS guidelines.
- Documented scheduled and completed visit types in ACO portal.
- Prepares and submits timely census and scheduling reports as required.
- Responds to client/employee problems as requested and represents the department in a courteous and professional manner.
- Ensures the rearrangement of missed visits and/or scheduling changes.
- Responds to patient/family member/caregiver concerns in a timely manner.
- Participates in in-service education, quality assessment activities and other staff meetings as required.
- Complies with all regulatory requirements, work rules and policies.
- Performs other related duties as assigned.
Position Qualifications
- High School Diploma or equivalent home health experience (one year).
- Scheduling experience preferred.
- Must be self-motivated, detailed, and accurate and have the ability to multitask.
- Able to work autonomously with minimum supervision in a fast-paced environment.
- Ability to plan and organize time effectively.
- Ability to deal effectively and courteously with clients, families, and co-workers.
- Demonstrates organizational skills.
- Demonstrates time management skills, and interpersonal skills.
- Effective verbal and written communication and data entry skills.
- Able to work in a team setting.
Job Type: Full-time
Pay: $48,000.00 - $52,000.00 per year
