Home Health Intake / Scheduler Coordinator

We are currently seeking a Full-time Home Health Scheduler to manage the Intake / Scheduler Coordinator in our agency in accordance with patients’ needs. The Home Health Scheduler is responsible for collaborating with the field clinical team. This person will be responsible for assisting with processing new referrals and coordinating with Director of Operations, Community Relations Liaison, Agency Supervisor and Nursing Supervisor.

Our team atmosphere coordinates with supporting our field staff, patients, clients and more. Office positions allow for personal and professional growth within the healthcare field.

Responsibilities:

• Responsible for assisting clinicians in coordinating and facilitating all clinical visits.
• Responsible for entering and maintaining the Intake patients in EMR system.
• Process incoming referrals received within a timely manner: verifying active insurance, staffing, and ensuring all proper documentation has been received
• Data entry into trackers and medical records systems
• Communicate with referral sources for further requests or status updates
• Coordinate with internal team on referral status updates
• Provide introduction calls to potential patients, follow up on quality assurance calls to ensure patient satisfaction, assist with Durable Medical Equipment, and input patient frequency and duration for clinicians
• Participating in the office On-Call rotation.
• All other duties as assigned.

Requirements:

• Minimum of one (1) year of experience in health care (hospice, skilled nursing, MD office, hospital)
• Ability to work alongside a team of professional and administrative personnel
• Autonomous, organized, assertive, flexible, and cooperative in performing job responsibilities in a fast-paced work environment.

Job Type: Full-time

Pay: $19.00 - $21.00 per hour
