Job Description
- Provides direct patient care in the patient’s home.
- Implements the plan of care initiated by the RN, PT, or OT.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Assists all clinicians or physicians in performing specialized procedures and duties, which include preparing equipment and materials for treatments, observing aseptic techniques as required, and assisting the patient in learning appropriate self-care techniques.
- Participates in the coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Counsels the patient and family in meeting nursing/therapy and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Identifies discharge planning needs as part of the care plan development and implements them prior to discharge of the patient, communicates D/C or ROC plan with the clinical manager.
