Position Title: Assisted and Enhanced Living Wellness Nurse

POSITION SUMMARY:
The Wellness Nurse is responsible for resident health assessments, development and implementation of individualized resident service/care plans, coordination of services with resident physician, pharmacy and ancillary providers and coordination of admissions, discharges, and hospitalizations on the Fieldstone and Stonebrook units. Additionally, the Wellness Nurse will provide training and oversight for team members in the provision of medication assistance/administration and assistance with health related tasks as allowed by regulation. Manage scheduling for Stonebrook Responsible Persons and C.N.A.s. Participate in on-call rotation on weekends and after business hours for Fieldstone and Stonebrook. Assist Program Director with development and facilitation of monthly team member in-services, as required by regulation. In collaboration with the Program Director, will work to ensure ongoing compliance with program policies and procedures and Executive Office of Elder Affairs and Department of Public Health regulations. POSITION DUTIES/ESSENTIAL FUNCTIONS:
- Adheres to and conveys the philosophy of supporting the dignity, privacy, independence, choice, and individuality of residents.
- Provides training and oversight to team members in the provision of medication assistance, health monitoring, and assistance with health related tasks as allowed by regulation.
- Provides training and oversight to ensure appropriate infection control procedures are followed by team members.
- Communicates with team members on all shifts on regular basis; participates in daily shift change reports.
- Conducts resident assessments; develops resident service/care plans.
- Conducts regular quality assurance reviews of team member documentation for medication/ care assistance provided to residents per service/care plan.
- Meets with resident and/or Health Care Proxy to review resident service/care plans per regulations.
- Provides service coordination with resident physicians, pharmacy and ancillary health care providers.
- Serves as main point of contact for resident medical and ancillary service providers.
- Coordinates and oversees the implementation of all new physician orders.
- Maintains resident charts and performs thorough and timely documentation of all resident incidents, changes in condition, behaviors and communications with providers and Health Care Proxy.
- Reviews all therapeutic diets and food allergies with dietary staff as needed to assure nutritional standards are met and are consistent with physician orders.
- Monitors the status of resident health conditions and initiates actions in response to changing needs; conducts reassessments of resident health needs as indicated.
- Coordinates provision of assistive therapy as necessary to maintain or improve residents’ functional status, with consultation from a licensed professional as needed.
- Assists the Program Director with developing and facilitating team member in-services as required by regulation.
- Reports qualifying incident reports to Program Director, EOEA and DPH as required.
- Conducts SAMM orientation with new Assisted Living team members.
- Conducts bi-annual Assisted Living team member education and competencies for resident care and SAMM.
- Facilitates clinical aspect of resident move in process.
- Completes Quality Assurance audits as scheduled.
- Conducts Assisted Living Introductory Visits with staff on all three shifts in required time frame.
- Responds to resident or team member concerns and reports them to the Program Director and/or others as appropriate.
- Provides health counseling/education to residents as needed.
- Shares AL on-call with Per Diem Wellness Nurse to respond to resident/team member issues.
- Participates in resident engagement as time allows.
- Performs all other duties as assigned.
- Must have a current RN/LPN license for the State of Massachusetts and be a graduate from an accredited school of professional nursing. Required
- Must have at least two years prior RN/LPN experience working with the elderly with dementia in either a residential/facility setting or in home-based care. Required
- Must be Certified Dementia Practitioner or willing to pursue certification. Required
- Must have a demonstrated ability to effectively communicate verbally and in writing.
- Must work well with others, have effective training and supervisory skills, and be able to identify and address problems effectively.
- Must be at least 21 years of age.
