Base Pay: $35.10 - $52.60 / hourJOB SUMMARY

Provides oversight for planning, implementing and evaluating Sweet Success Program guidelines. Provides individualized, evidence-based diabetes self-management education, training, and support to individuals and groups according to the Scope of Practice, Standards of Practice, and Standards of Professional Performance for Diabetes Educators (AADE) in a clinic-based setting.

JOB DUTIES AND RESPONSIBILITIES
  • Responsible for ongoing development of the clinic’s diabetes management program
  • Ability to effectively educate patients in a group setting or one-on-one
  • Performs initial diabetes self-management education assessments with patients, collaboratively develops individualized plan of education, learning objectives, and ensures completion of plan
  • Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods.
  • Collaboratively develops an individualized follow-up plan with each program participant
  • Evaluates effectiveness of education services provided by measuring attainment of learning objectives
  • Document assessment data, educational plan, educational services provided and evaluation results in each participant’s educational records
  • Performs insulin and non-insulin injectable medication training
  • Utilizes a team approach to provide services and collaborates and communicates with team members when needed
  • Participates in development of education materials
  • Responsible to track data necessary to measure outcomes and meet standards required to maintain program accreditation
  • Contributes to the ongoing quality improvement process to measure program strengths and weaknesses
  • Maintains a minimum of 8 hours of diabetes-related continuing education annually and ensures team members meet education requirement as well
  • Serves as resource for MD/NP and residents by providing up to date research and information when needed
  • Communicates with Maternal Fetal Medicine Consultant as needed for proper patient management
  • Performs other related duties as assigned or requested in a professional manner

Assisting the Clinician and Daily Activities

  • First line for resolving patient issues/complaints.
  • Dispenses samples and patient assistance medications and provides patient education on these.
  • Nurse only visits.
  • Nurse advice – telephone and face to face.
  • Obtain previous records, collecting patient history, preparing new patients, etc.
  • Resource for VC, patients, MA, LVN, etc.
  • Administers injections and diagnostic test/referrals as ordered, including completing lab and other services ordered by the clinician.
  • Performs age appropriate screening as needed or as directed by clinician.
  • Performs accurate nursing assessment of patients and initiates emergency measures as appropriate.
  • Assures efficient patient flow by ongoing assessment of clinician schedule while working with other staff to minimize patient waiting time.
  • Follows SCHC standards for Standard Precautions when working with potentially infectious materials (blood or body fluids).
  • Schedule patients appropriately when needed.
  • Alerts Clinical Director or designated person to needed supplies/equipment.
  • Reports any safety hazards.
  • Performs other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
  • Knowledge of nursing process, scope of practice and standards of care
  • Ability to pass a written medication quiz
  • Basic computer proficiency in Windows environment
  • Typing 35 WPM or greater
EDUCATION & EXPERIENCE
  • Completion of an accredited RN program
  • Current California RN license upon hire and maintain current
  • Current CPR/BLS certification upon hire and maintain current
  • CDAPP Sweet Success training per CDAPP requirement, Annually
PREFERRED QUALIFICATIONS
  • One year inpatient and/or outpatient experience as consultant and previous experience working on an L&D/PP/Nursery hospital unit
  • Spanish speaking skills
  • Typing 45 WPM or greater
