SalarySentara is currently hiring a Registered Nurse-Primary Care Centralized Services Full Time with Day Shift as a Registered Nurse with Sentara; you can excel in your career. This position's primary duties include assessing/admitting patients, rounding checks, giving medications, talking with family members, and being responsible for the overall care of your patients. This role focuses on improving the health status of and caring for individuals with chronic conditions through the promotion of preventative care and wellness, education on the management of chronic disease processes through self-care, patient-centered care planning, discharge planning, health coaching, care coordination, and transitional care management processes. This position is Monday-Friday from 8:00 AM-4:30 PM, no weekends, no on-call. This position is hybrid; three days are spent in the physician's office and two days are spent at home.
Duties Include:
- 15 patient contacts per day via phone, office, etc.
- Post hospitalization Follow-Up (Hospital Admissions and ER Visits)
- Patient Education (via Telehealth or in the office)
- Vaccine Clinics
- Annual Wellness Visits
- Chronic Care Management
- Preventative Care and Wellness Promotion through registry work
