As an MDS Coordinator, you will coordinate documentation of MDS 2.0 and Care Planning through collaboration with all members of the Interdisciplinary Team.

Requirements for an MDS Coordinator:

• Current valid and unrestricted LPN or RN licensure in the state of Ohio.
• Current knowledge of gerontological nursing.
• Significant knowledge of licensure, certification, and other regulatory bodies of long term care.
• Excellent clinical and assessment skills.
• Ability to work independently and meet timeframes.
• Relevant computer and software skills

Responsibilities for an MDS Coordinator:

• Coordinates documentation of MDS 2.0 and Care Planning through collaboration with all members of the Interdisciplinary Team.
• Is able to link MDS 2.0 and Resident Assessment Protocol information to the care planning process.
• Is responsible for computer entry all resident assessment data.
• Initiates and completes comprehensive care plan for all residents.
• Attends and participates in continuing education programs and all mandatory in-services.
