PRIMARY PURPOSE:
The Navigator functions as a member of the multidisciplinary team of care providers who reflect a culture of cooperation, enthusiasm and respect. The Navigator is responsible for coordinating the appropriate care of the patient based on an evaluation/assessment of the patient (telephonically or in-person) that matches the patients desires to the most applicable program that the Surgery Department offers. This includes education, continuity of care and support through all phases of diagnosis, treatment and follow-up care for patients in seeing that maintain a high level of engagement and participation in the program that is critical to their success and the goals that they desire. The role is mainly clinical, but also one that incorporates business development and a high level of patient engagement skills.
EDUCATION:
Required: Graduate from an accredited nursing program
YEARS OF EXPERIENCE:
Required: 3+ years of RN experience
Preferred: 1 or more years of Labor & Delivery experience
REQUIRED SKILLS AND KNOWLEDGE:
In-depth knowledge of issues and treatment related to Surgical and other available options for patients based on clinical assessment (telephonically) to determine the best course of action for the patient.
Knowledge of resources within the community. Working knowledge of all office equipment including computer, keyboard, printer, fax, etc.
Excellent communication skills; computer literate. Ability to work efficiently in (EPIC) EMR system.
Managing a clinic schedule for first visit consults and follow-up appointments, performing physical exams, interpreting lab results, prescribing and managing medication, completing preoperative evaluations, and ensuring patients fulfill their insurance requirements for surgical services.
Screening patient has been called for additional diagnostic views and is very anxious. “Navigator” will offer support and re-assurance.
Communication barriers between patient/families, disciplines, departments and community agencies. Encourage collaboration and bring the interests of the patient and the collaborative team together. Demonstrated ability to work successfully in a team-based decision-making culture.
Demonstrated ability to work independently resulting in effective outcomes and on-time performance.
Experience in planning and coordinating multi-disciplinary communications strategies, strategic initiatives, and events.
Must respond and follow through to requests from customers promptly.
Must work carefully and precisely with attention to detail.
Must utilize resources wisely. Performs duties willingly and with initiative. Shares necessary information so co-workers can do the same. Cooperates with other departments and work groups.
LICENSES AND CERTIFICATIONS:
Minimum: Must have a valid driver’s license for travel between office sites. Current registration to practice nursing in the state of Kentucky. Obtains required CEU’s, including all state required courses to maintain license.
BLS
Active Kentucky RN licensure
Direct communication with a patient in a language other than English requires the associate to complete an assessment to become Qualified Bilingual Staff (QBS). The associate is required to complete CBLs and future assessments to maintain QBS status.
