DEPARTMENT SUMMARY: The Los Angeles LGBT Center’s Department of Health Services is one of the nation’s largest and most experienced providers of LGBTQ-specialized primary care and related services, Our mission to help LGBTQ people lead healthier, happier lives is supported by our comprehensive array of client-centered services including primary care, mental health, psychiatry, sexual health, pharmacy, case management, nutrition, clinical research, housing services and a food pantry. The Center is one of a handful of Federally Qualified Health Centers in the nation specializing in serving LGBTQ people.
JOB SUMMARY: Under the supervision of the Manager of Nurse Case Management Services, with the collaboration of the Director of the Audre Lorde Health Program (ALHP), the ALHP RN manager will be intrinsic in expanding and managing clinical services for LBTQ women and non-binary folks at the LA LGBT Center. The ALHP RN Manager will work with the ALHP programmatic staff to recruit and maintain patients from this demographic, including identifying and attending outreach events, cultivating relationships with community-based and other agencies servicing this population, and developing and implementing processes for patient intake and ongoing clinical support. The ALHP RN manager will help create and oversee clinical programs specific to LBTQ women and non-binary folks including expanded reproductive health and gynecologic services. The ALHP RN manager will collaborate with the QI team to support efforts specific to ALHP patients including the clinic’s breast/chest cancer and cervical cancer screening initiatives. The ALHP RN Manager will collaborate with the Director of the ALHP and programmatic staff on the creation of health education materials, ALHP-focused social media campaigns, and with MarCom to update and expand the ALHP section of the Center’s website.
ESSENTIAL FUNCTIONS:
• Liaisons with ALHP programmatic staff to identify and attend outreach events for health education and patient recruitment.
• Establishes relationships with organizations serving LBTQ women and non-binary folks with high psychosocial needs including the criminal/jail system and other community-based organizations and agencies.
• Develops and oversees processes for establishing care at the Center for new ALHP patients including conducting intensive intakes with patients to solicit medical and social histories and identify and address additional psychosocial needs.
• Increase patient retention through the provision of ongoing nursing case management to newly established patients.
• Provides clinical support to staff gynecologist in care coordination and in the preparation and follow-up for gynecologic and reproductive health clinical encounters and procedures.
• Working with CMO, provides clinical support in the development and implementation of new reproductive health initiatives.
• Develops patient education and health promotional materials on ALHP specific issues.
• Provides education sessions to patients and community members regarding ALHP specific health issues.
• Collaborates with MarCom and the Director of the Audre Lorde Health Program on the development and implementation of ALHP social media campaigns to expand the ALHP section of the website.
• Follow guidelines to verify patient eligibility for referral authorization, e.g., Medi-Cal/Medi-Care status.
• Provides clinical and administration support for the clinic’s breast/chest and cervical cancer screening initiatives participating in QI work groups.
• Communicates appropriately and tactfully with staff, consultants, patients/clients, significant others, and community members to reduce conflict and increase client/patient satisfaction whenever possible.
• Interacts in a respectful and professional manner with health services staff and patients.
• Exercise appropriate judgment and decision-making skills.
• Participate in weekly ALHP team meetings, staff meetings, agency meetings, planning meetings, and other meetings as needed.
• Comply with requirements of OSHA, HIPAA, and other applicable regulations.
• Other duties as assigned.
JOB QUALIFICATIONS AND EXPERIENCE:
• Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people.
• A passion for the Center’s work and its mission to make the world a better place for LGBTQ people.
• Passion for working with LBTQ women and non-binary folks and a deep understanding of the barriers this population faces in accessing comprehensive care.
• Minimum 3 years experience in direct patient care, preferably in an ambulatory care setting.
• Strong clinical knowledge regarding medical, reproductive, and sexual health issues specific to LBTQ women and non-binary folks, including contraception, fertility options, menstrual health, and menopause.
• Commitment to a clinical practice that is body-and-sex-positive, trauma-informed and grounded in the tenets of harm reduction.
• Excellent verbal and written communication skills, including strong organizational, detail, and interpersonal skills.
• Excellent computer skills and knowledge of Microsoft Office, word processing, Electronic Health Records, and other software systems.
• Ability to be self-motivated and dedicated to initiating and completing complex tasks.
• Ability to handle multiple tasks, be detail-oriented, and maintain strict confidentiality of medical information.
• Able to work well independently with minimum supervision.
• Bilingual skills, a plus (English/Spanish fluency preferred).
• Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment.
