Job Purpose: To coordinate incoming referrals and assist with coordination of care.

Duties:
The Intake Coordinator Home Health is responsible to ensure incoming referrals for care are appropriate and meet regulatory requirements. Coordinates visit schedules to ensure appropriate care is delivered and timely.
- Reviews incoming referrals for appropriateness and ensures compliance with all requirements for HH services
- Ensures the consistent availability of all care/services offered by the agency.
- Manages and coordinates the provision of quality home health services to patients from admission to discharge.
- Participates with the physician, referral sources and professional staff in decisions regarding patient eligibility and suitability of home health services.
- Coordinates nursing schedules to ensure appropriate, timely delivery of care in compliance with Physician orders and Regulatory requirements.
- Evaluates, and regularly re-evaluates the needs of the patient; initiates, develops, implements and makes necessary revisions to the patient's plan of care. Assesses the patient's continual care needs.
- Works cooperatively with the Clinical Manager on staffing, referral sources, physician and client concerns/needs.
- Applies knowledge of Federal/State rules and regulations, ACHC accreditation standards, and other regulatory requirements to ensure compliance and quality standards for the patients.
- Recommends and arranges for resources needed to provide care/services.
- Reviews all incoming referrals for new patients to ensure all pertinent/needed information is obtained. Queries physician/referral source when needed for further information when needed.
- Assures the availability of qualified clinical supervision of direct care personnel 24/7.
- Participates in agency quality assurance activities designed to improve quality and continuity of patient care.
- Informs the physician and other personnel of changes in the patient's needs and outcomes of interventions as indicated, or delegates as appropriate.
- Utilizes case management approach and referring to other services as needed
- Complies with organization's policies and procedures
- Maintains proficiency with agency's electronic medical record
- Participates in on-call rotation
- Provides direct patient care as staffing requires
- Performs other duties as assigned

Skills/Qualifications:
- Degree in Nursing from an approved school of nursing as determined by the Board of Nursing, required. LPN or RN.
- At least two (2) years of home health care experience, preferred
- Management/supervisory experience helpful but not required
- Must be a licensed driver with an automobile that is insured in accordance with state
and/or organization requirements and is in good working order
- Excellent observation, verbal and written communication skills, problem solving skills,
basic math skills; nursing skills per competency checklist
Certificates, Licenses, Registrations:
- Degree in Nursing
- Current licensure in state and CPR certification
