Rn Hospice Case Manager

RN Hospice Case Manager(full-time, direct hire position with excellent full benefits package and competitive base/bonus compensation structure)

Responsibilities:

• Completes initial comprehensive and ongoing assessment of patient and family to determine Hospice needs
• Admissions of patient for services and development of the initial patient/family Plan of Care
• Coordination of services with the patient, family, and hospice interdisciplinary team members
• Ongoing clinical supervision of Licensed Practical Nurses and Home Health Aids

Requirements:

• Valid Registered Nurse (RN) license
• RN Diploma, Associates or B.S. degree in nursing
• Minimum 1 year of Hospice agency experience required
• Valid Driver’s License, auto insurance and reliable transportation
• Excellent observation, problem solving and communication skills
• Compassion and integrity

Job Type: Full-time

Pay: $99,000.00 - $104,000.00 per year
