Position: Registered Nurse (RN) - Home Health
Responsibilities:
- Provide skilled nursing care to patients in their homes.
- Conduct comprehensive assessments and develop individualized care plans.
- Administer medications, treatments, and procedures as prescribed by physicians.
- Monitor patient vital signs, symptoms, and overall health status.
- Educate patients and families on disease management, medication adherence, and self-care techniques.
- Collaborate with interdisciplinary healthcare team members to coordinate patient care.
- Document patient assessments, interventions, and outcomes accurately and timely.
Requirements:
- Valid Registered Nurse (RN) license in the state of practice.
- Minimum of 2+ years of clinical experience, preferably in Home Health or a related setting.
- Strong assessment, clinical judgment, and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work autonomously and as part of a multidisciplinary team.
- Reliable transportation and willingness to travel to patient homes.
