Rn Case Manager-Home Health Visits

Amazing Care Home Health Services is currently hiring RN Case Manager for Loveland, CO Home Healthcare Visits.

Coverage Areas: Loveland/Fort Collins and Surrounding areas.

Please note: Pay rate is PER VISIT not hourly.

Job Requirements:

• RN License Required.
• Home health experience required
• Ability to work independently.
• Excellent documentation skills with a solid grasp of computer usage.
• Provide effective communication to patient/family, team members, and other health care professionals.
