Under general supervision of the Heart Failure Program Director and the Heart Failure Clinical Medical Director, the RN coordinates patient care/services from the inpatient setting to an outpatient setting in order to improve patient outcomes,
Essential Duties and Responsibilities:
Serves as a liaison between the patients, families and healthcare team during hospitalization and the post- discharge period
Communicates to Cardiologist, HF Director, and hospital staff regarding abnormal findings in the evaluation of patients,
Works with the hospitalists, cardiologists, and the HF Director in assessing readiness for discharge and risk for readmission,
Responsible for the guidance and coordination of care for patients and their caregivers through the continuum of care for both inpatient and outpatient,
Provides individualized information, support and education,
Works with Case Management and Heart Failure Care Team to coordinate appointments for consultations and physician visits,
Participates in interdisciplinary nursing rounds for inpatients -discharge planning process,
Outpatient responsibilities for patient education,
Responsible for PI and other metrics to determine effectiveness with all level of care providers to optimize the patient care experience,
Maintains a thorough knowledge and understanding of evidence based/best practice,
Revises and monitors educational materials to insure that they are coordinated, understandable and meet the program standards,
Protects patient confidentiality by promoting professional staff communications,

Reporting actual and potential errors, as well as hazardous conditions,
Identifying opportunities to standardize processes and 'error proof' systems that will lead to increased safety,
In addition, participating in safety education programs and root cause analyses as required,
Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population,
Assesses and interprets patient age specific data and provides appropriate, age specific treatment,
Heart Failure Nurse Navigator
Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards,
Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation,
Responsible for the care and maintenance of department/entity equipment and supplies,
Attends meetings as required, and participates on committees as requested,
Performs other related duties as identified,
Qualifications:
Note: Other combinations of formal education, training and experience may be considered,
Minimum Education Bachelor's Degree
Minimum Experience 3 - 5 years nursing experience
Registration Registered and current RN license in NJ, BLS and ACLS required
Prefer Cert/Registration Certification in appropriate nursing specialty; Cardiology nursing experience preferred
