- Inspects nursing stations and medication carts in accordance with consultant guidelines.
- Makes rounds and provides medication observations to nurses and nursing supervisors to enhance patient outcomes; evaluates and make recommendations regarding the quality of care.
- Plans and coordinates Pharmacy training programs in support of nursing centers, including formal or informal patient and family education, IV infusion therapy, Proxy Caregiver training, CMA training, and BLS.
- Collaborates with nursing staff and prescribers regarding the administration of patient medications and IV access; assists nursing centers with infusion therapy when needed; may assess patient needs for IV access and/or recommend access devices and products.
- Monitors, collects, and analyzes data to identify medication related opportunities for quality improvement to ensure State and Federal requirements are met including standards for TJC accreditation.
- Maintains current knowledge of medications and IV access through continuing education and professional organizations and by keeping current of new products and services.
- Communicates regularly with the Manager of Clinical Services, Consultant Pharmacists, Dispensing Pharmacists, nursing center staff, and others to discuss customer satisfaction and potential service enhancements.
- Collaborates with the consultant pharmacist to assist the center in identifying, evaluating, and addressing pharmacy-related concerns and issues that affect patient care, medical care, quality of life, and/or regulatory compliance.
- Collaborates with the consultant pharmacist to ensure centers remain compliant and aligned with state and federal regulations as it relates to pharmaceutical services.
- Collaborates in the development of IV policies and procedures.
- Participates in center quality improvement and drug safety initiatives, as needed.
- Assists in the education and adoption of pharmacy programs and service offerings.
- Travel overnight as needed.
- Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
- Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
- Performs other duties as necessary to ensure the success of the System.
