The RN Case Manager is a Registered Nurse (RN) who has graduated from an accredited school of nursing and is currently licensed to practice in the state where currently practicing. The RN Case Manager also supervises home health services to homebound patients in their place of residence in accordance with attending physician orders and plans of care and strives to provide the highest quality of care.
What you’ll be doing:
- Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
- Developing an individualized plan of care for each client that incorporates analysis of data and current scientific findings.
- Providing interventions with a focus on achieving realistic client outcomes.
- Reporting changes to update the plan of care to reflect progress towards goals and outcomes.
- Administering medications as needed.
- Maintaining client records showing systematic assessments, planning interventions, and evaluations.
- Initiating referrals as needed to other health team members.

What we’re looking for:
- A passion to serve and help others live their best lives possible.
- Registered Professional Nurse , a graduate of an approved school of professional nursing who is currently licensed as a registered nurse in the state of agency operation.
- Five (5) years experience as a RN with (3) Three years in home health program and maintain CPR certification while employed in a clinical role.
- Access to reliable transportation.

- In-depth understanding of Medicare Conditions of Participation and payment mechanisms for home health.
- Strong organizational, planning, problem solving, decision-making and leadership skills required.
