HOW YOU’LL MAKE AN IMPACT
Each day you will engage with our ACT clients, helping them to have purposeful and fulfilling lives. You will get to know the clients by assisting with treatment plans that are centered around a client’s own personal strengths, needs, and aspirations for the future. Each team member participates in an on-call team approach to ensure our services are customized to individual needs and staff will respond 24/7.
The duties may include but are not limited to:
- Coordination and assessment of the Client’s medication management plan so that optimal health is maintained.
- Implement services in the treatment plan for each recipient under clinical supervision.
- Complete assessments and maintain client documentation.
- Provide community based services
- Provide direction to ACT staff
This position would include some evening and weekend shifts and an on-call rotation.
WHAT YOU’LL NEED
- 2-year nursing degree from an accredited professional nursing program or certificate of completion from a 12-month accredited practical nursing program.
- Registered Nurse in the state of Minnesota without encumbrance.
- The Mental Health Practitioner: RN shall meet the qualifications defined in Minnesota Statute 245.462 subdivision 17 or Minnesota Statute 256B.0623 subdivision 5.

- Clearance from DHS and sexual exploitations screening.
- Valid Driver’s License and acceptable driving record.
- CPR and First Aid certification required.
- Must be at least 18 years of age.
