Under guidancefrom members of management, The Respite Unit Care Coordinator delivers andoversees care management services for medically and/or socioeconomicallycomplex patients in accordance with defined goals, a multi-disciplinaryplan of care, and established policies and procedures. The Nursing Care Coordinatoris responsible for facilitating care coordination for the Medical RespiteProgram with The Waterfront Rescue Mission, reviewing medical respite referralsfor appropriateness, and planning and evaluating nursing services for clientsin the program. In this role, the NursingCare Coordinator will practice independently within the scope of the RNlicensure, and collaboratively within the multidisciplinary team, providingnursing support to Medical Respite clients and appropriate communitylinkage. Performs all reasonably related duties as assigned.

Education/Experience:
- Current Registered Nurse (RN) licensure in states of Florida & Alabama practice
- Two years of professional nursing experience in a clinical setting preferred.
- Previous work in a community health center or homeless shelter preferred.
- Knowledge of the principles and skills to provide patient care and treatment.
- Thorough knowledge of homelessness, harm reduction, mental illness, and substance abuse preferred.
- Ability to administer health care and a variety of treatments (within scope) and medications as directed.
- Possess the ability to make independent decisions when circumstances warrant such action.
- Proficient in the use of computers, such as Microsoft Office, Electronic Practice Management (EPM) and/ or Electronic Health Record (EHR).
- Demonstrates strong interpersonal skills and excellent communication skills both written and verbal, as well as exhibiting strong organizational skills and work ethic.
- Demonstrates a professional demeanor when responding to patients/clients.
- Must have the ability to react quickly and calmly in an emergency and recognize symptoms of common mental illness and make appropriate referrals for treatment.
- Ability to work with diverse populations and all levels of people within the workplace.
- Ability to work independently in a fast-paced environment and be organized with extreme attention to detail.
EssentialJob Duties and Responsibilities:
- Effectively performs clinical tasks including, but not limited to, client assessment, referral review, monitor and assess client's health, client education, and assisting clients negotiate the health care system.
- Manages the delivery of health services provided in the Medical Respite Program.
- Facilitate multi-disciplinary team meetings to include Medical Respite staff and case management.
- Assists with development of and maintains Medical Respite policies, procedures, and standards.
- Identifies training needs for Medical Respite staff, participates in planning, developing, and evaluating training.
- Participates in on-going continuous quality improvement activities to evaluate effectiveness of the Medical Respite Program.
- Collaborate with the Mission in creating and implementing client care plans.
- Identifies program needs for growth and assists the Director of Ministry with implementation of changes to meet Medical Respite needs.
- Assist in building capacity in the Medical Respite Program.
- Regularly seek to update and maintain a current level of knowledge relative to professional practice and maintain continuing education requirements necessary for licensure.
- Some travel may be necessary for training & temporary duty.
- Other duties as assigned by immediate supervisor or other member of management.
Physical Demands:
While performing the duties of this job,the employee is regularly required to sit and use hands to manipulate; to speakand listen. The employee is occasionally required to stand; walk; reach withhands and arms; stoop, kneel, crouch, or crawl and taste or smell. The employeemust occasionally lift and/or move up to and sometimes over 50 pounds. Specificvision abilities required by this job include close vision, distance vision andcolor vision.
