Job Purpose: To provide a full array of case management services to clients residing in our Residential Treatment programs and to ensure individual treatment needs are being met and established goals are being addressed.
Primary Duties:
- Case Management: Attend court hearings and Administrative Case Reviews (DCFS), maintain regular contact with collaterals, attend school meetings, schedule and lead clinical staffings, actively participate in the Treatment Team, responsible for obtaining necessary items for the youth's file, create and maintain current information on clients face sheets, coordinate admissions, schedule and coordinate youth visits, maintain contact with employers to obtain performance updates and to communicate information (i.e. Supervision Plan) and provide accurate and detailed information to the Treatment Team and milieu staff.
- Paperwork: Document all mental health services/case management services provided, provide required assessments, summaries, and reports according to DCFS guidelines and agency protocol, complete other paperwork as needed
- Other: Transport youth as needed, participate in the on-call rotation as assigned, other duties as assigned
- Minimum: Associate's Degree or 60 credit hours towards a Bachelor's Degree in a Human Services field plus two years' experience in Child Welfare. Must have a valid Illinois driver's license.
- Desired: Bachelor's Degree in Social Work with previous Case Management experience.
- Miscellaneous: Must pass and maintain all pre-employment drug screens, background clearances, and driving clearances.
- Skills: Excellent interpersonal skills, ability to multi-task, organized, detail oriented, basic computer skills, excellent communication skills both written and verbal
