The successful Home Health Aide or Personal Care Aide will direct care and support to residents of our Assisted Living Facility under the supervision of the Enriched Living Manager or LPN.

ESSENTIAL FUNCTIONS:
- Provide direct care and support for the grooming of the resident according to the resident's care plan.Grooming includes:
- Bathing.
- Shampooing hair.
- Eyes and glasses care.
- Oversee resident as needed to manage good oral hygiene.
- Shaving.
- Ear care and/or hearing aid care.
- Nail care (filing as requested by resident family).
- Morning, evening and night care.
- Provide for the maintenance and rehabilitative needs of the residents according to the resident's
care plan.Maintenance rehabilitative needs include:
- Assuring safe mobility and ambulation. Encouraging active exercise through formal on site classes or 1:1 walks in/out of the building.
- Active exercise.
- Provide direct assistance to the RN Nurse Manager through the gathering, reporting and documenting of information requested by the residents' physician.This includes the following:
- Temperatures (oral).
- Collection of urine and stool for testing.
- Weight.
- Change in resident functional abilities.
- Medication assistance and recordkeeping.
- Assistance with one person transfer (on/off toilet, in/out of chair, in/out of bed).
- Assistance with unmanaged incontinence (includes toileting schedule, clothing management, hygiene).
- Assist with oxygen, ostomy and catheter care.
- Provide for a safe, clean environment in the resident's apartment.This includes:
- Make resident's bed daily (changes linens when needed and/or weekly) and empties trash daily.
- Straighten kitchenette area as needed.
- Maintain a clean bathroom on a daily basis, provides input to housekeeping to maintain a neat apartment.
- Communicate pertinent information to RN Nurse Manager or LPN on duty at the change of shift.
- Work cooperatively with other departments.
OTHER FUNCTIONS:
- Responsive to resident needs and preferences.Consider resident satisfaction a priority.
- Assist residents in getting to activities, beauty salon, therapies or other appointments as necessary.
- Contribute to the ongoing documentation in the resident's care plan.
- Provide for a pleasant dining experience for the residents.This includes:
- Assisting residents to and from the dining room if necessary.
- Any duty in the dining room: service or cleanup as requested.
- Ensure resident privacy during personal care and at other appropriate times.
- Treat all residents with dignity and respect.
- Maintain confidentiality of resident information.
- Provide friendly, helpful and courteous service to everyone with whom contact is made.
- Perform all assigned tasks in a safe, sanitary and efficient manner in accordance with our established policies and procedures.
- Attend and/or complete required inservices.
- All other job duties as assigned.

EXPERIENCE:Previous experience as a home health aide or working with the elderly is desirable.
RELATION TO OTHER POSITIONS:Daily contact with and ability to interact courteously and tactfully with staff, residents, family members, volunteers and visitors.
PHYSICAL AND MENTAL DEMANDS:Sits rarely.May be required to lift (and/or move) 50-100 pounds.Frequent twisting, bending, squatting, reaching, walking, pushing and pulling large objects, kneeling.Physical work intensity level 4.
WORK ENVIRONMENT:Works in well-lighted, ventilated areas, such as resident apartments, nursing station as well as throughout the building.
ADVERSE WORKING CONDITIONS:May be exposed to infections, disease and disagreeable odors.May be subject to hostile and emotionally upset residents and family members.
VARIABLES:Required to work weekends and holidays.May be asked to work beyond normal working hours.May be asked to work on a different shift or unit than assigned.
