We are a growing company that is looking for MAs to join our team. This is a work-from-home position. We empower patients to maintain their well-being in the comfort and familiarity of their own homes. We ensure our patients have the means to obtain medications, understand what medications to take and how to take them and avoid unnecessary ER visits and hospitalizations. This position is a telephonic, 8 to 5 M-F work-from-home position.

Responsibilities of the MA Include but are Not Limited to:
Providing the best complex care management to our members; the frail, elderly, chronically ill, and/or functionally challenged. Our main goal is to help the member live their best lives in a safe home environment and prevent hospitalizations through education and support. Must be able work independently. Must be proficient with self-scheduling, and independently able to maintain a weekly schedule.
Typical Duties of the MA Include:
· Identify risks, gaps in care and prevent unnecessary hospitalizations and emergency room visits by developing an individualized interdisciplinary plan of care for patients and families to follow in order to achieve life-long well-being in the home.
· Educate post-hospital members on discharge instructions, medication reconciliation, follow up appointments, and any resources to help the member transition successfully from the hospital setting back to their home environment.
· Educate patients on preventative health care.
· Collaborate with other members of the “care team” including patient’s physicians and other medical professionals.
· Educate on resources to assist with medication reconciliation and understanding of medical conditions.
· Engage patients on a collaborative relationship which empowers the patient to manage his or her physical environmental and psycho-social health issues, to improve and maintain lifelong well being and remain at home.
· In-home, hospital and facility visit with patients (as deemed).
· Provide patients and their family/caregivers with educational material and coaching.
· Completion of required paperwork and documentation within mandated time frame.
· Maintaining professional care clinician responsibilities by setting appropriate boundaries, complying with expectations and productivity standards, and seeking managerial consult, as needed.
· Monitor vital signs via Remote Patient Monitors, contact patients when vital signs alert and contact the physician when vitals are out of parameters or trends are noted.
· Comply with all on-boarding, annual and other mandatory training as assigned.
· Maintain HIPPA compliance.
· Performs other related duties as assigned by management.
Requirements of the MA:
· Must have an active, unrestricted license in the state where you reside.
· Must have intermediate computer skills and proficiency with Microsoft Word, Outlook and Excel.
· Must have a valid driver's license, and a smartphone.
· High Speed Internet Connectivity.
· Must have own computer

Pay: $12.00 - $15.00 per hour
Expected hours: No more than 40 per week
