Remote Care Manager Iii - Ny- Rn/Lmsw/Lcsw

Attention all licensed RN/LMSW/LCSWCare Managers in NY State! Are you bilingual in Spanish, Russian, Bengali, Mandarin, Cantonese, or any other language? Do you have experienced in MLTC and NY Medicaid policy? Do you have a passion for creating care plans for adult and geriatric patients? If so, we have an exciting opportunity for you! Our client is seeking a Care Manager to join their team, with the flexibility to work remotely from anywhere in the US as long as you are licensed in NY State (RN/LMSW/LCSW). However, please note that the position will not be able to convert to perm if you are not in the NY tri-state area. Required Experience: 5-7 years of clinical experience with Case Management experience.
As a Care Manager, you will be responsible for outreach calls, person-centered care plan reviews, and coordinating care for a caseload of 225-230 members. Additionally, you must be bilingual in Spanish, Russian, Bengali, Mandarin, Cantonese, or any other language. The ideal candidate will have previous remote work experience, be self-motivated, tech-savvy, and able to work independently to complete a minimum of 7 successful calls per day.
This is a non-negotiable position, and the candidate must be licensed in NY State as either an RN, LMSW, or LCSW with knowledge of UAS Assessment. The rate for this position is up to $50.00 per hour depending on experience and will require a laptop, keyboard, mouse, headset, dual monitors, and a docking station.
