Job description

BASIC FUNCTION:
The Registered Professional Nurse cares for patients by diagnosing and treating actual or potential health problems in the ambulatory setting without discrimination to age or gender through implementation of the nursing process. Coordinates care of other disciplines, delegates tasks to other nursing staff members, supervises care and evaluates results. Maintains competence in clinical practice and assists in the development of nursing practice standards through participation in professional committees and organizations.
EDUCATION AND EXPERIENCE REQUIREMENTS:
The Registered Nurse requires a license to practice in New York State. The Registered Nurse is expected to complete a variable orientation, duration of 4-6 weeks. In addition, ambulatory experience preferred.

PHYSICAL REQUIREMENTS:
The position is located indoors under normal working conditions. Should be able to push/pull 35 lb., lift/move 15 lb. from floor to table, be able to perform moderately difficult manual manipulations such as using a key board, writing and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers. Mobility requirements may include the ability to sit at a computer terminal or work station for a prolonged period of time in addition to being able to squat, stand and walk for a reasonable length of time and distance. Sensory requirements include the ability to articulate and comprehend the spoken English language in addition to being able to read the English language.
FUNCTIONAL PERFORMANCE STANDARDS AND RELATED MEASUREMENT CRITERIA:
Performance Standards Measurement Criteria
A.
The RN will complete patient/family assessment using subjective and objective data sources.
1.
Evaluates progression in life milestones for the following types of male or female patients/family:
  1. Infants
  2. Children
  3. Adolescent
  4. Pregnant Clients
  5. Adults
  6. Elderly

2.
Identifies patient/family assets and liabilities.
3.
Identifies physiological, social, psychological and spiritual needs of patient/family.
4.
Evaluates assessment data and establishes nursing diagnosis.
5.
Prioritizes patient needs and selects diagnosis which can be addressed in the ambulatory setting.
B.
The Registered Nurse will develop a NCP based on patient assessment. Particular attention is given to developmental stages of the patient/family.
1.
Develops a nursing care plan based on nursing diagnoses, input from other disciplines and involvement of the patient and/or family.
  1. Uses nursing practice standards and protocols
  2. Establishes realistic measurable goals
  3. Establishes time frames for goal achievement for the visit and over time of the relationship.

2.
Identifies nursing interventions to meet goals.
3.
Individualizes interventions according to patient assets and liabilities.
C.
The Registered Nurse will deliver patient care based NCP.
Skills include:
  1. HIV counseling
  2. Medication administration
  3. Use of Tympanograph
  4. Use of audioscope
  5. Snellen eye testing
  6. Venipuncture
  7. Laboratory testing
  1. Blood glucose monitoring
  2. Occult blood testing
  3. urine dipstick testing
  4. urine pregnancy testing
  5. beta strep testing
  1. Obtaining cultures
  2. Assisting with primary care procedures
  3. Suture removal
  4. Sterile dressing application
  5. CPR
  6. Spirometry
  7. Electrocardiogram machine
  8. Other

1.
Delivers nursing care based on the nursing care plan and patient response to interventions.
2.
Delivers nursing care in accordance with accepted standards of nursing practice.
3.
Delegates patient care tasks to non-R.N. staff members.
4.
Observes, interprets, reports and documents care given directly and by other nursing staff members.
5.
Communicates observations through established channels.
D.
The Registered Nurse will evaluate patient care.
1.
Assesses and documents the response of the patient/family to interventions.
2.
Assesses level and timeliness of goal achievement.
3.
Modifies the plan of care as required to meet patient needs.
4.
Monitors own practice and the
practice of ancillary staff in the use
of equipment, conformity and
appropriate disposal of hazardous material.
E.
The Registered Nurse will effectively communicate the patient's plan of care to all members of the health care team.
1.
Maintains communication through timely documentation in the medical record.
2.
Communicates with physicians and other health providers on changes of patient condition and patient responses to medical therapy within a timely manner
3.
Determines equipment and supply needs of patients and delegates responsibility for delivery and maintenance of these items.
4.
Organizes nursing care assignments appropriate to the education and skill of available staff members and patient needs and communicates task responsibilities accurately.
5.
Communicates periodically with assigned staff members regarding patient care needs and responses.
6.
Collaborates effectively with nursing management, colleagues, other personnel, patient and family members to coordinate care delivery.
F.
The Registered Nurse will provide patient education focusing on wellness health maintenance and restoration.
1.
Assesses continuing care needs and plans for anticipated needs.
2.
Implements plans and collaborates with other providers of the interdisciplinary team.
3.
Assesses patient teaching needs as part of the initial and ongoing needs evaluation, sets goals and develops teaching plans consistent with the patient's educational level and learning readiness.
4.
Implements and/or coordinates the teaching plan, evaluates results and adjusts the plan as necessary
G.
The Registered Nurse will assume responsibility for education and self development.
1.
Maintains competency in nursing practice, and keeps up with new knowledge and advances in the profession, and completes the required mandatory inservices.
2.
Attends continuing education programs pertinent to maintenance and development of nursing expertise related to the area of chosen practice and keeps record of own inservice.
3.
Participates in nursing quality assurance and other nursing and interdisciplinary research and study activities as required.
4.
Incorporates nursing knowledge in nursing practice and utilizes time and resources available to maximize knowledge and skills.
5.
Functions within the standard of practice, policy, procedures and guidelines of the nursing department.
6.
Participates in effort to continuously improve unit operations.
7.
Recognizes job relationship and functions effectively within the organizational structure.
H.
Adheres to the policies and procedures as outlined in the Exposure Control Manual regarding:
  1. Universal Precautions
  2. Bloodborne Pathogen
  3. Disposal of Needles, Syringes and Sharps
  4. Cleanup Procedures for blood and body fluid spills
  5. Storage, Handling and Disposal of Medical Waste

1.
Demonstrates knowledge and understanding of the policies and procedures outlined in the Exposure Control Plan.
2.
Practices safe work techniques and utilizes proper engineering controls to minimize the risk of bloodborne pathogen exposure.
3.
Maintains clean and safe work area free of biohazardous waste.
I.
Appropriate Personal Protective Equipment is utilized when performing a task or responsibility associated with this position which represents a potential occupational risk of exposure to bloodborne pathogens.
Appropriate PPE's shall include but not be limited to gloves, gown, goggles, laboratory coat, face shields or masks.
1.
Demonstrates knowledge of the appropriate level of personal protective equipment to be used in relation to the specific tasks and responsibilities.
2.
Degree of compliance as measured by personal observation of immediate supervisor/manager.
3.
Degree of compliance as measured by feedback from other supervisors/ managers.
J.
Adheres to hospital policies and procedures related to:
  1. mandatory education
  2. annual health assessment
  3. licensure verification

1.
Continuing education records will be completed and forwarded to Human Resources annually regarding mandatory educational requirements.
2.
Complete annual health assessment form as required, maintain current inoculations.
3.
Current license as required by N.Y. State will be forwarded to Human Resources upon renewal or issuance.

Salary Range: $29.44-$44.16/hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
