Job Description:
HealthForce is a medical staffing agency that focuses on placing contract nursing professionals across the state of Virginia. We are currently working with the Fairfax-Falls Church CSB to provide a Behavioral Health RN to work with their Assertive Community Treatment (ACT) / Intensive Case Management Team (ICM). There are three offices: one in Alexandria, one in Reston, and one in Annandale. The successful candidate will need to be able to reliably commute to each location based on the needs of the program and do community visits if needed.
This is an exciting opportunity to provide comprehensive Behavioral Health nursing care to individuals diagnosed with mental illness. Registered Nurse will deliver therapeutic nursing and behavioral health interventions under the supervision of nursing supervisor. This position is responsible for clinical and primary care issues including the following: serves as the liaison to community resources, pharmacies, and other community medical providers. Provides and gathers information for clinical collaboration with physicians/medical staff and the mental health treatment team regarding medical support for clients. Provides therapeutic medical and behavioral health interventions, consultation, and education about medical issues to non-medical team members. Performs health screening, nursing assessments, and supports the integration of behavioral and physical health.
Applicant must be computer literate and be able to learn and utilize Electronic Health Record; and must maintain accurate, timely and detailed electronic documentation in client records that meets state, local and federal performance contract expectations.
Duties of this position include:
1. Complete the Nursing Assessment.
a. If the individual does not have a Primary Care Physician, assist the individual in obtaining one by referring them to an Affordable Health Care Clinic in the community, such as Neighborhood Health.
2. Schedule an initial PCP appointment for clients
3. Provide Health Literacy Counseling per Federal/State/Agency regulations.
4. Request recommended labs, such as those needed for metabolic screening and clozapine levels.
5. Assess individuals to determine if a referral for urgent medical care should be made.
6. Educate and encourage individuals to complete an annual physical, dental, and eye examination.
7. Contact the primary care physician directly to facilitate and coordinate care.
8. Support clients with some case management needs. You will not be assigned a caseload.
9. Request hospital records to include admission history, discharge summary, lab and test results, consultations, and all recommendations for medical follow-up.
10. Scheduled follow-up appointments and consulted with the medical team for complex medical conditions to ensure correct care coordination as deemed necessary.
