POSITION SUMMARY:
Coordinate and manage care for elderly participants enrolled in the Family Care Program.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Prepare initial assessment to include physical assessment, obtaining past and current medical history, functional abilities, and nutritional status.
- Ongoing risk management and health assessment according to guidelines set by Milwaukee County Dept. of Aging, clients' needs, and level of care.
- Care plan development that needs to be revised every six months.
- Health education for members and/or family and/or caretakers
- Coordination of services including but not limited to medical, nursing, and home health needs, services, equipment, and supplies.
- Serve as consultant to the social worker regarding all health care needs and concerns.
- Medication management
- Demonstrates Basic computer knowledge, Basic knowledge of excel, word, and Google forms.
- Demonstrates the ability to multitask and prioritize work.
- Demonstrates the ability to work as a team as well as independently.
- Demonstrate the ability to address criticism in an appropriate manner and develop constructive outcomes.
- Demonstrate sustained effort and enthusiasm in the quality and quantity of work.
- Demonstrate the ability to establish and maintain positive, effective working relationships with staff, clients, and the community.
- Demonstrate the ability to follow and communicate effectively, both orally and in writing.
- Demonstrate excellent human relations and interpersonal skills.
- Complete accident reports when necessary.
Qualifications
QUALIFICATIONS:
Must have excellent interpersonal and communication (written and spoken) skills and the ability to work in a multicultural environment. Exceptional organizational and multitasking skills. Must be dependable, flexible, detail-oriented, and have basic computer knowledge. Previous experience working with the elderly and disabled is seen as an advantage. A bilingual Spanish speaker is highly preferred, but not required.
REQUIREMENTS:
Bachelor's Degree in Nursing. Must be certified as a Registered Nurse with the State of Wisconsin. Must have a valid driver's license and insured vehicle.
WORKING CONDITIONS:
Office setting with the use of basic medical and office equipment. Some exposure to inclement weather. Frequent travel for home visits.
