Responsibilities: - Provide case management services to patients in a healthcare setting - Assess patient needs and develop individualized care plans - Coordinate and collaborate with healthcare professionals to ensure continuity of care - Monitor patient progress and make necessary adjustments to care plans - Educate patients and their families on healthcare management and resources - Document patient information accurately and maintain confidentiality Requirements: - Valid Registered Nurse (RN) license - Experience in pediatrics, behavioral health, or acute care preferred - Knowledge of managed care principles and practices - Ability to perform diagnostic evaluations and interpret results - Proficient in vital signs monitoring and assessment - Strong understanding of physiology and medical terminology - Excellent communication and interpersonal skills - Detail-oriented with strong organizational abilities This is an exciting opportunity for a Registered Nurse Case Manager to make a difference in the lives of patients.
