JOB TITLE: Registered Nurse-Heart Failure Clinic
GENERAL SUMMARY OF DUTIES: The RN Coordinator functions interdependently with the physician to coordinate independent advanced assessment, planning, implementation, and evaluation of the care for a select group of patients, in collaboration with physicians, surgeons, mid-level providers, patient/family and other ancillary departments and outside agencies.
ESSENTIAL FUNCTIONS: (This list may not include all of the duties assigned.)
- Assess health status of patient including state of wellness and compliance with care plan.
- Develops care plan.
- Follows Heart Failure Institute protocols.
- Adjusts medications following CHF medication protocols and in collaboration with the referring cardiologist.
- Provides monitoring and continuity of care between physician visits.
- Instructs patient/family regarding medications and treatments as dictated by HIPAA guidelines. Educates patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.
- Triages patient and family telephone calls and provides consultation within 24 hours.
- Responds to emergencies, performs BLS, and implements ACLS as needed.
- Serves as role model and resource. Attends and participates in required department and company meetings.
- Maintains patient confidentiality.
PAY RANGE: $38-48/hr to start (dependent upon experience and education)

Education:
Minimum Graduate of an accredited school of nursing
Experience:
Minimum - 5 years of professional nursing experience in a cardiology setting with an emphasis on caring for patients with congestive Heart Failure
Certificate/License: Current Alaska Registered Nursing license required
Current BLS and ACLS certification
CHFN certification preferred
