Registered Nurse | Community Hospice Nurse
- Shift/Hours: Monday - Friday, First Shift, 9:00 AM-5:00 PM
- Location: Manhattan/ Brooklyn, NY (1 position)
Experience
- Registered professional nurse with 2+ years experience in either home healthcare or Hospice nursing.
- Registered nurse currently licensed in the state of NY
- Baccalaureate Degree in nursing from an approved program or
a baccalaureate Degree in nursing from a non-approved program
supplemented by content that can be equated to the approved program. - Knowledge of Computer operation, and computer-generated clinical records experience preferred.
- Demonstrated commitment to Hospice philosophy of care.
- Unless ALL assigned patients can be served by using public transportation, must have a reliable automobile, a valid driver’s license, and the minimum state-required liability automobile insurance.

Registered Nurse | Community Hospice Nurse Position Summary
As a Registered Nurse, you will be responsible for providing nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary Plan of Care. Supervises Home Health Aides. Supports the provider relations and Quality Improvement activities of the organization.
Registered Nurse Responsibilities Include
- Receives patient assignments from the Hospice Nurse Coordinator.
- Assumes responsibility for a patient/family that includes assessing, planning, implementing, and evaluating care within the first 24 hours of patient/family admission:
a) Ensures that the plan of care reflects the patient’s current conditions and
needs to be based on the initial and all subsequent assessments.
b) Nursing visit notes address and reflect those issues that are on the plan of
care. New conditions or changes are reflected on the Plan of care through the
hospice IDT plan of care update. Assumes leadership in all modifications of
care planning and ensures all interventions are implemented as
planned/prescribed.
- Initiates communication with attending physicians, other hospice staff members, contract providers, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
Nursing Home Facility: Promotes and maintains a collaborative relationship with facility staff in the delivery of an integrated plan of care within a skilled nursing facility. See Policy # 274
Inpatient Care Facility: a) Ensures that there is a q-24 hour clinical note written by facility staff or hospice staff justifying the patient needs a continuation on this level
of care. Absence of such notation will be brought to the attention of
the unit supervisor and the HOSPICE NURSE coordinator & or Q.I. RN.
b) Ensures that all facility hospice records are kept up to date with care
plan updates, assessments, and other medical records/documentation
concerning the hospice patient/guardian.
- Maintains regular communication with the Hospice Nurse Coordinator to review care.
- Maintains regular communication with the Medical Director and attending physician concerning patient/family care.
- Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the Plan of Care.
- Maintains up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated and changes are reflected as they occur.
- Accepts responsibility for coordinating the physical care of the patient by teaching primary caregivers, volunteers, and employed caregivers, and/or by providing direct care as appropriate.
- Inform the Hospice Nurse Coordinator of unusual or potentially problematic patient/family issues.
- Attends Interdisciplinary Team Meetings and other patient conferences as deemed necessary by the Hospice Nurse Coordinator.
a) Takes the initiative in motivating other team members actively during discussions and presentations of the Hospice patient’s care issues and
treatment plan.
- Provides appropriate support at the time of death.
- Shares in providing 24-hour seven-day-a-week coverage to patients and families.
- Makes Hospice Aide / Home Health Aide assignments, prepares written instructions for the aide, and supervises the aide in the home.
- Participates in hospice orientation and ongoing education programs.
- Demonstrates familiarity with policies of the agency and state(s) and federal regulations about nursing services.
- Participates in committees and special projects and other related duties as assigned.
