Job Description
The Nurse Care Coordinator is a RN Registered Professional Nurse provides direct assessment, evaluation and teaching to meet the needs of individual and families within their home and communities. The Nurse Care Coordinator is an RN with emphasis on community health education/experience.
This is a homecare position not nursing home, you will not be looking after patients. The main responsibility of the role of the RN will be to supervise the aides and developing plan of care, as well as overseeing medication management weekly- little to no skilled care. You will be case managing 60+ pt as a homecare nurse not a nursing home clinician. Aides will do the direct care. You will perform assessment visits every 6 months, supervisory/aide instructing visits as needed for any new aide on a case as well as annual aide evaluations. Your cases are in the building and SI community. We reimburse travel times for her community cases. You will also attend to incidents and improve performance through aide’s reorientation as needed.
QUALIFICATIONS:
- Registered Professional nurse in the state.
- At least two years’ experience as a Registered or practical Nurse with one year administrative/supervisory experience in community health
- Completion of a baccalaureate degree in nursing or an associate degree with at least 1-year supervisory experience
RESPONSIBILITIES:
- Completes assessments of patient and family to determine home care needs at regular established timelines.
- Provides a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluates patient nursing needs and processes information to comply with all Federal, State and City regulations, as well as all agency Policies and Procedures.
- Develops the plan of care with patient and family; establish goals based on nursing diagnosis and incorporates therapeutic and preventive nursing actions.
- Provides health care instructions to the patient/family as appropriate per assessment and plan.
- Instructs, supervises, and evaluates home health aide care provided as stipulated by law and regulation.
- Prefills medication boxes as per MD’s order.
- Communicates with the physician regarding the patient’s needs, plan of care and reports and changes in the patient’s condition; obtains/receives physician’s orders as required and prepares clinical notes.
- Submits all documentation on a timely basis.
- Investigates and reports all incidents and complaints as per agency policies.
- Ensures that arrangements for equipment and other necessary supplies and services are available.
- Anticipates the needs of the patient and recommends changes to Doctor and family as appropriate.
- Assures that all logs are maintained as required by Agency.
- Participates in on-call duties as defined by the on-call policy.
