Registered Nurse ADMISSIONS for VNA Home Health & Hospice is an organized and timely Registered Nurse who completes the admission in the patient's home or in a health care facility on the OASIS platform in a timely manner. The Home Health Registered Nurse ADMISSIONS acts with exceptional autonomy while remaining connected with the multi-disciplinary team including Clinical Manager, Case Managers and others to support the team and patients with all needs. The Home Health Registered Nurse ADMISSIONS at VNA is offered a unique schedule of Monday thru Sunday one week; then OFF the following Monday thru Sunday (7 days on / 7 days off) and be paid for 80 HOURS! Just what would someone do with every other week off???

The Work of the Home Health Registered Nurse ADMISSIONS Includes:
- Provide agency information and assesses immediate or emergent needs of patients and families by arranging appropriate care at the time of admission. Assesses potential patients/families for admission into the programs of VNA.
- Explains and obtains consent forms as needed for each program.
- Establishes initial plan of care, obtains medication and DME orders as needed.
- Documents patient information obtained from the assessment, all medications, DME and Plan of Care into the computer system in accordance with current policy.
- Provides teaching to family members, caregivers, and nursing home personnel as needed.
- Provides direct nursing care to patients in order to maintain the highest level of care for the patient under the direction of the primary care physician.
- Communicates with members of the team regarding patient/family needs
- Develops positive and effective relationships with physicians, referral sources, insurance case managers, nursing homes, patients, families, and coworkers.
- Uses resources efficiently and prudently.
- Schedules or manages his/her time effectively.

Registered Nurse ADMISSIONS Must Have:
- Graduate of an accredited school of nursing, Bachelor's Degree Preferred. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) year of home health care and/or hospice care experience preferred.
- Current licensure in Missouri and Kansas, as well as CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Management experience not required. Responsible for supervising nursing aides.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist to include but not limited to lab draws, IV care and administration, ostomy care, wound care and Wound Vac implementation, foley catheter insertion and care, Pleurex Catheter draining and care, nephrostomy care, trach care
- Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
