PRIVATE DUTY HOME CARE AGENCY
Position: Registered Nurse----PRN
TITLE: RN SupervisorAccountable to: Agency Director
Position Summary
The RN Supervisor provides oversight to client services – including assessment, admission care planning, supervision and discharge of all clients. The RN supervisor coordinates care with other professionals and obtains the physician authorization as indicated. The RN supervisor participates in the orientation and training of all clinical staff, validates skills and provides continued in service training and clinical staff supervision. The RN supervisor participates in the Balance HomeCare Services Performance Improvement Plan including quarterly record review, complaint investigation and incident management. The RN supervisor develops new policies as needed, in accordance to Virginia Department of Health Professionals Guidelines and standards of Care and submits to the Agency Director for final review and governing body/owner approval. The RN supervisor may act as Director in the absence of the Agency Director.
Qualifications· Current, valid Registered Nurse License for state of Virginia· Must have a negative criminal background check· Must have negative TB screen · Must have current and valid drivers license · Preferable to have at least one year of hands-on assessment in a homecare settingDuties1. Complete initial in home assessments of referred clients. To include at a minimum:
A) In-home Assessment of client and service needs and adequacy of home environment (CC 03).
B) Obtains In-home aide orders as indicated and/or PA/PCS authorization (CC 10).
C) Discusses and explains client’s rights and responsibilities with clients at admission (CC 06).
D) Develops, supervises and updates a client Plan of Care (CC 05) to include type of services needed, frequency of services needed, days services are needed, and time of day services are needed. Delegates tasks to nurse aides for in home aide services.
E) Provides explanation to potential client of the scope of services that this agency provides and which services this agency will or will not be providing to them (according to the individual client care plan needs) upon approval from their primary/attending physician.
F) Completes and records assessment, client’s plan of care for physician’s approval, including PCS PA / PACT or physician’s order for plan of treatment if indicated.
G) Return upon completion all documents to agency premises.
2. Conducts client in-home supervisory visits at least every 90 days (ADM 11) and documents such findings in 5 days. Return all completed forms to agency premises per policy. Completes Nursing Supervision (CC 11) per standards, including evaluations of all clients.
A) Any and all changes to be noted in clients current Plan of Care (CC 05).
B) Client’s condition including physical, mental, emotional, and living condition
C) Quality of service that is currently being provided by the client by this agency.
D) Document any comments or complaints from client or client’s family (CC 09).
E) Document any items requested by client or that you may feel that the client may be in need for assistance such as: walkers, wheel chairs, shower seats, hospital beds, diabetic shoes, etc. complete any items request form for the attending physician to complete and approve.
F) Document any safety concerns regarding client’s current condition die to mental, physical, emotional, or living conditions. Document any suggestions in resolving the concern.
G) Return all documentations to the agency premises within 5 days.
3. Reports to the agency director and records any and all complaints made to RN by any persons or family members of persons receiving services from this agency, or any persons providing services to other persons of this agency (CC 09). These complaints will be documented in writing dated and signed by RN and the agency director. Follow up or actions taken to resolve the issue will be documented dated and signed by all persons involved with in a reasonable amount of time.
4. Completes quarterly record review (ADM 12); participates in Performance Improvement Plan (ADM 10), including providing incidents, complaints and participation in Annual Agency Evaluation (ADM 11).
5. Provides in-home staff supervision and validation in-home staff skills (HR 05).
6. Establishes and maintains all skills evaluation and competency test for all active and potential in-home aides providing.
A) Competency of each in-home aide to be documented before being assigned any job assignments.
B) Quarterly evaluations of all in-home aide job performance.
C) Annual evaluation to be conducted in the client’s in-home aides.
D) Documentation of each evaluation and all competency tests are to be returned to agency premises and placed in personnel files.
7. Continuous availability during the hours for which the agency will be providing services to clients, by means of a cell phone. *NOTE: A special code of three numbers (911) in a text message. This code will be acknowledged by returning the call ASAP! If the incoming text does not contain the three-digit code then the call will be returned at your next availability. All other contacts will be made by home phone contacts, cell phone contacts, person-to-person meeting, etc.
8. Provides assistance in overseeing the agency policies are being followed by all agency staff including owner/director.
9. Provides assistance in other areas that the director may require your advice or guidance.
10. Develops procedures for new care activities not listed on current care (ADM 06). Determines with BON care tasks appropriate. Researches standards of care to develop procedures.

Seeks in-service programs to increase competency in the job requirement’s specific area of practice and/or expertise. Upholds Virginia DHP continued competency requirements.
Maintains license
Documents according to the established agency procedureDocuments according to agency and payer policies. Submits all paper work into office per agency policy.
Maintains a clean and safe working environmentFollows all standards for Balance HomeCare Services Inc precautions, infection control, blood and airborne pathogens, and agency policies when reporting to assignments.
Registered Nurse AssessmentFollows all policies of agency, NC BON, DHSR and of the Division of Medical Assistance. Completes assessment on all clients and follows documentation requirements.

Delegates tasks and supervises aides and in home nurses.
Assists in orientation and education of Home Care Staff

At moderate risk for occupational exposure
There may be emergency situations where the RN Supervisor will have to react quickly with sound judgment. The scope of duties described in this document may not include all duties to be performed by the RN Supervisor and is subject to change.
