We are seeking a highly organized and detail-oriented Referral Coordinator to join our team. As a Referral Coordinator, you will be responsible for managing referrals and ensuring that patients receive the necessary care in a timely manner. As a member of a multi-disciplinary team, the position includes processing and coordinating patient referrals from multiple sources. This includes communicating with members of the team as well as, practices, providers, and outside agencies.

- Process patient referrals from multiple sources (Fax, EMR, calls, and email)
- Completing, logging and assigning referral requests

To excel in this role, you should possess the following skills:
- Microsoft office products (Teams, Outlook, Excel)
- Strong organizational skills and attention to detail
- Two or more years in a healthcare setting (practice or ambulatory setting preferred)
- Two or more years working within an electronic health record (EMR or EHR)
