The Referral Coordinator is responsible for scheduling specialist appointments and tracking and following up with health center patients. The Referral Coordinator will report to the Practice Manager.
Essential Duties and Responsibilities:

  • Schedule referral appointments for all center patients.
  • Verify insurance eligibility utilizing the various portals.
  • Complete required referral forms.
  • Provide directions to referral appointments, as needed.
  • Call and send a reminder letter for future appointments.
  • Contact patients to remind them of the appointment the business day before the appointment.
  • Call the place of referral and determine if the appointment was kept on the day of the appointment if that information is not in EMR. If the appointment was not kept, re-initiate the process.
  • If the results are not in EMR, call the specialist or facility to receive a copy of the result or consultation letter to reconcile the order in EMR.
  • Enter completed referral data into the EMR, including the appointment time, data, and location.
  • Scan specialist reports into EMR.
  • Track and reconcile all referrals.
  • Required to support other sites as staffing or workflow dictates.
  • Perform other duties as assigned, which may include scanning.

Front Desk Coverage as Needed:
  • Verify appointment and demographics.
  • Verify insurance by utilizing the appropriate insurance portal
  • Collect and scan insurance cards and ID into the EMR
  • Check in patients via EMR
  • Upon checking out, print After Visit Summary (AVS) to give to patients.
  • Schedule follow-up appointments
  • Provide transportation support, including Septa pass and or parking validation if applicable.
  • Assist with appointment confirmation calls for the provider schedules.

PHMC Compliance Responsibilities:
  • Understands and adheres to PHMC compliance standards as they appear in the PHMC Code of Conduct, Whistle Blowers, and Conflict of Interest Policies.
  • Keeps abreast of all pertinent federal, state, and PHMC regulations, laws, and policies as they presently exist and as they change or are modified.
  • Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job.
  • Comply with the Department of Public Health (DPH), The Joint Commission, and other accreditation and regulatory agencies standards.
  • Adhere to all PHMC Policies and Procedures.
  • Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module.

JOB REQUIREMENTS:

Skills:
  • Knowledgeable about specialist referrals.
  • Data entry experience, particularly with practice management system
  • Knowledge of medical terminology
  • Must have a pleasant manner.

Experience:
  • One year experience working either as a Medical Receptionist or a Referral Coordinator in a primary care setting.

Education:
  • High School Graduate or GED

Physical Demands:
Position requires standing 2/3 of the time, walking 2/3 of the time, requires sitting under 1/3 of the time, use of hands to finger, handle or feel 2/3 of the time, reaching with hands and arms, under 1/3 of the time, stoop, kneel, crouch or crawl under 1/3 of the time, talk or hear over 2/3 of the time. The position requires lifting up to 10 lbs up to 1/3 of the time.
Work Environment:
Moderate noise (examples: business office with computers and printers, light traffic). Exposure to blood-borne pathogens requires use of personal protective equipment.
Salary:
  • Based on education and experience.
