Emerest Health of Pennsylvania excited to be adding an Home Care Referral Eligibility Coordinator to join our team in Natrona Heights, PA.
About the Position:
- The Eligibility Manager's purpose is to accelerate the eligibility process of new leads that will be future clients of Emerest. They should be well acquainted with the portfolio of services Emerest has to offer. They will work under direct supervision of the Administrator. Moreover, a vital part of this position requires building long-term relationships with families, access agencies and the Department of Social Services. Some participation in community events such as fairs, marketing events is also part of the position may be necessary. This role will be in person.
Accountabilities:
- Customer satisfaction is paramount. By taking ultimate responsibility for all assigned customers and by closing loops
- Identifies opportunities to grow business with existing clients
- Enhances customer satisfaction by resolving questions/complaints in a timely manner
- Assists in optimizing performance by improving communication with Production and Sales
- Manages projects within client relationships, working to carry out client goals while meeting company goals
- Responsible for record keeping while adhering with all existing HIPPA regulations
- Assesses, identifies, qualifies, quantifies, and prioritizes all potential referral sources in the territory with a specific focus on community based, grass root referral sources and some professional referral source development as warranted.

Qualifications:
- Case Management experience is required.
- Bachelor's Degree; Preferred: 3-5 years of experience OR Diploma and 10+ years of relevant experience.
- Extensive knowledge of MS Office, Windows, Teams, etc.
- Outstanding interpersonal and organizational skills
- Ability to multi-task and strong organizational skills
- Excellent relationship building skills: ability to work with other people to achieve company and departmental objectives and goals.
- Ability to multi-task and prioritize competing tasks on a daily basis
