Job as a Peer Support Specialist/Recovery Coach:
The Peer Support Specialist/Recovery Coach works with the care management team in providing a seamless continuum of care by maintaining regular and constant communication with patients to provide a set of non-clinical activities that engage, educate and offer support to patients. The Peer Support Specialist/Recovery Coach will help patients to remove personal and environmental obstacles to health care access and help them remain in recovery.

Job as a Peer Support Specialist/Recovery Coach:
The Peer Support Specialist/Recovery Coach works with the care management team in providing a seamless continuum of care by maintaining regular and constant communication with patients to provide a set of non-clinical activities that engage, educate and offer support to patients. The Peer Support Specialist/Recovery Coach will help patients to remove personal and environmental obstacles to health care access and help them remain in recovery.
Responsibilities as a Peer Support Specialist/Recovery Coach:
- Facilitate peer lead support groups.
- Serve as a mentor for patients by demonstrating ways to become empowered and self-responsible.
- Providing care management and care coordination services for patients in the clinic. Responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements;
- Coordinating care and serve as patient advocate;
- May also be responsible for conducting individual, and family educational sessions, various duties related to coordinating treatment and discharge activities, as needed;
- Coordinates in a timely manner, issues or activities relevant to the team, and/or to the team's relationship within the facility or treatment milieu; and,
- Perform other duties as assigned
Qualifications
Skills/Qualifications as a Peer Support Specialist/Recovery Coach:
- High School Diploma preferred, but not required
- 1-2 years of experience working in behavioral health or substance use disorder recovery setting preferred.
Licenses/Certifications:
- Certified in Intentional Peer Support (CIPSS) or must be a Certified Recovery Coach.
