DESCRIPTION
The City of Long Beach, Department of Health and Human Services has an immediate opening for a full-time, unclassified, at-will Public Health Professional III to serve as the Reach Team Mental Health Coordinator in the Homeless Services Bureau. Reporting to the Homeless Services Officer, the Public Health Professional III position plays a key role within the REACH Team to provide strength-based street outreach to encourage people experiencing homelessness to engage in available services. This position is located at the Multi-Service Center, 1301 West 12th Street, Long Beach, CA 90813.
PLEASE NOTE: In accordance with the State Health Care Worker Vaccine Requirement, the candidate placed in this position is required to be vaccinated against COVID-19 prior to their first day unless a medical or religious accommodation is approved by the City of Long Beach's Human Resources Department. Proof of vaccination (including booster) will be required at the time of your pre-employment physical.
EXAMPLES OF DUTIES
  • Responds to high priority street outreach requests to complete MENTAL HEALTH assessments and coordinates appropriate clinical services, including behavioral health involuntary holds when needed.
  • Provides case management and housing navigation services to persons with severe physical, MENTAL HEALTH or substance use needs.
    • Participates in case conferencing and care coordination for higher barrier clients to assist in navigating local and county homeless and health care systems.
  • Navigate the Long Beach Coordinated Entry System to link to housing resources, including utilizing the Homeless Management Information System (HMIS) and other data platforms to document all client interactions in a timely manner.
  • Support Multi-Service Center non-clinical staff with MENTAL HEALTH assessments and evaluations when necessary.
  • Fosters and develops collaborative working relationships with the Long Beach Police and Fire Departments and other social service agencies.
  • Provides housing retention and location services to prevent re-entry into homelessness
  • May provide direct transportation to people experiencing homelessness to assist in accessing services (in City of Long Beach Vehicles).
  • Participates in community education regarding REACH services and the City's response to people experiencing homelessness including ride-alongs and community presentations.
  • Will supervise existing street outreach staff by providing direction regarding clinical interventions and assessments when in the field.
  • May assist in coordination of outreach activities; and,
  • Performs other related duties as assigned.

REQUIREMENTS TO FILE
  • Master's Degree in Social Work, Marriage and Family Therapy, or a closely related field (*proof required).
    • Program management experience in a public agency beyond the minimum requirements may be substituted for education on a year-for-year basis.
  • Licensed Clinical Social Worker or Licensed Marriage and Family Therapist Certification required
  • One (1) year of professional experience providing MENTAL HEALTH support or care coordination for a social or human services program.
  • Valid motor vehicle operator license.
    • Required to obtain a California motor vehicle operator license within 6 months of employment if license is out of state. DMV K4 driver license information report printout required during onboarding if selected for position.

DESIRABLE QUALIFICATIONS:
  • Experience conducting street outreach and clinical service delivery to people experiencing homelessness.
  • Ability to work evenings, nights, weekends, holidays, and on-call during emergencies as needed.
  • Experience working with the Homeless Management Information System (HMIS)
  • Bilingual skills (Khmer, Spanish, Tagalog, and/or Vietnamese)

  • Ability to work effectively with a wide variety of people and populations by consistently exercising tact, good judgment, and a proactive, problem-solving focused style.
  • Ability to deescalate and resolve conflict effectively.
  • Ability to work within deadlines, manage multiple assignments, and set appropriate priorities.
  • Proficiency in the use of computers, including email, internet, and office productivity software (e.g. Microsoft Word, Excel, and PowerPoint).
