Compensation & Benefits Package
- $58,240 - $66,560 Depending on Experience
- Reimbursement for Insurance
- Paid Time Off: 2 Weeks Per Year for First Year

Job Responsibilities
- Monday-Friday Schedule
- No On–Call
- Review OASIS documentation for accuracy.
- Submit daily completed OASIS documents that have been coded, locked, and certified.
- Receives concurrent telephonic report on Oasis visits/documentation for review of patient’s clinical information to ensure accuracy and efficiency.
- Creates/coordinates a frequency and duration plan of care with a clinician.
Job Requirements
- Licensed LPN or RN in the State of Illinois
- Home Health Experience Required
- Understanding of the Chicago Area
- Comfortable with Kinnser EMR
- Familiar With Home Health Documentation
