Generations at Oakton Pavillion, a 24-hour Skilled Nursing Facility located in Des Plaines, IL is seeking a Registered Nurse to be our Complex Patient Care Facilitator (QA Nurse). The ideal candidate will need to have had a few years of Respiratory (Vent/Trach) Care experience as well as some Dialysis, Infection Prevention and complex Wound Care experience.

Generations at Oakton Pavillion is offering a starting salary of $104,000 per year. This position is part of a matrix reporting structure that includes the VP/Director of Respiratory Services and the facility Director of Nursing.

The Complex Patient Care Facilitator responsibilities include:
- Coordinate care for complex, high acuity patients through collaborating with the nursing, respiratory, dialysis, wound care, infection prevention, and therapy teams
- Educate patients and families on the care plan by providing details related to treatments, procedures, medications and set realistic recovery goals
- Continually evaluate/measure short and long term outcomes of care and sharing results with the interdisciplinary team
- Participate in Care Plan meetings, coordinating information among the interdisciplinary team, monitoring doctor's orders and medication prescriptions
- Monitor all care management assessments and interventions
- Round regularly with physicians to ensure that care is proceeding appropriately and update care plan if needed
- Follow-up to ensure that ordered tests and treatments are proceeding efficiently and that results are available to physicians on a timely basis
- Consistently communicate plan of care updates/changes/progress with patients, families, physicians and interdisciplinary team
- Refer to Social Worker/Case Manager for complex psychosocial integrations in the plan of care
- If needed, collaborate with nursing and physicians, to coordinate the transfer of the patient to another facility if a higher level of care is required
- Ultimately responsible for the quality of care of patients on this unit

Requirements
Requirements for the Complex Patient Care Facilitator Role:
- Must be a Registered Nurse, with an unencumbered license in the State of Illinois
- Must have experience working with high acuity patients, including Respiratory patients with vents and/or trachs
- Hospital, LTACH or Skilled Nursing Experience working with complex patients needing respiratory and/or Dialysis, Wound Care and Infection management
- Absolutely must have critical analytical, problem-solving skills; acute attention to details
- Be able to communicate effectively with patients/families in such a manner that they clearly understand the complexity of care that will be provided
- Collaborate well with peers, department heads and staff
- Have the ability to resolve conflicts of opinions in a professional manner
- Ability to work independently and collaboratively with others under pressure
- Have an outstanding bedside manner
- Exude a compassionate, caring demeanor
