Essential Duties and Responsibilities:

• Provide intensive, solution focused treatment strategies to adult clients enrolled in mental health skill building service.
• Facilitates care coordination with other service providers to support clients in their areas of need.
• Prepare treatment plans and updates on progress monthly and quarterly.
• Maintain two cases billing on average 10hrs per week.
• Collaborate with clinical manager in regard to assessments being conducted, submission to kepro, and case management needs.
• Provide case management services to families that are awaiting approval from kepro
• Document, utilizing progress notes case management services provided to client’s awaiting approval from kepro.
• In providing case management services the staff should bill no less than 3hrs per week and no more than 12hrs per week as deemed necessary by the clinical manager.
• If authorization is approved aid in appropriate transition of client to permanent counselor
• If authorization is denied conduct appropriate discharge and closure with the family
• Attend all staff meetings and trainings.
• Assist program manager with chart audits and quality assurance on programmatic level.
• Adhere to all policies and procedures and complete other tasks deemed necessary by the program manager/clinical manager for the successful operation of the program.
• Assessing client's mental health status through interviews or observations and developing Individual Service Plans and assisting in making referrals to appropriate services.
• Provide crisis intervention services to clients who are a danger to themselves or others.
• Accurate, descriptive, and timely documentation must be maintained and submitted for all client interactions.
• Prepares written discharge summaries and plans.
• Participate in weekly Supervision, trainings, and staff meetings

Job Type: Contract

Pay: $25.00 - $30.00 per hour

Expected hours: 10 – 40 per week
