JOB SUMMARY

  • Responsible to integrate, coordinate, and monitor the Active Treatment Programs for their assigned case load in accordance with current federal, state, and local standards, guidelines and regulations, facility policies and as may be directed by the Administrator and/or designee.

QUALIFICATIONS

  • Be an individual of who holds at least a bachelor’s degree in Human Services field, i.e., Psychology, Sociology, Speech Communication, Gerontology, Education Counseling, Human Development, Anthropology, Rehabilitation Counseling, etc.
  • At least one (1) year of experience working directly with persons with mental retardation or other developmental disabilities.
  • Must possess current CPR certification or become certified within ninety (90) days of employment.
  • Must be able to read, write and speak the English language in an understandable manner.
  • Must possess the ability to make independent decisions when circumstances warrant such actions.
  • Must possess the ability to deal tactfully with residents, family members, visitors, personnel, government agencies/personnel, and the general public.
  • Must possess the ability and willingness to work harmoniously with other personnel.
  • Must have patience, tact, a cheerful disposition, and enthusiasm.

PHYSICAL REQUIREMENTS

  • Must be able to move intermittently throughout the workday.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately to assure that the requirements of this position can be fully met.
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, family members, personnel, and support agencies.
  • Must be in good general health and demonstrate emotional stability.
  • Must be able to relate to and work with the ill, disabled, young, emotionally upset and at times, hostile people.
  • Must be able to lift, push, pull, and move equipment, supplies, resident in excess of fifty (50) pounds throughout the day.
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including exposure to the (HIV) AIDS and Hepatitis B viruses.

ESSENTIAL FUNCTIONS

  • Implement, evaluate, direct, and monitor the established Active Treatment Programs for respective case load, in accordance with established policies.
  • Identify problems, develop, and implement solutions with the assistance of the Administrator and/or designee.

Review the department’s policy and procedure manual, Job Descriptions, etc., at least annually for revisions and make recommendations to the Administrator and/or designee to assure client needs are met.

  • Assure that all personnel follow established policies and procedures, using the disciplinary process as appropriate, and informing the Administrator and/or designee.
  • Assist in developing a written plan of care for each resident that identifies the Active Treatment needs of the resident.
  • Attends and actively participates in IDT conferences.
  • Report all concerns, complaints made by the resident, guardian, or their family to the Administrator and/or designee.
  • Meet the Director of Nursing, Assistant Director of Nursing, nursing personnel and review the 24-hour report on a regularly scheduled basis to assist in identifying and correcting problem areas, and/or the improvement of services.
  • Review communication and appointment logs on a daily basis.
  • Make daily rounds to assure that personnel are performing required duties, and to assure that appropriate policies and procedures are being followed.
  • Act as a liaison between facility, resident’s families/guardians, and school personnel.
  • Maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department.
  • Plan, conduct and participate in In-service Education Programs. Demonstrate (hands on) regarding the resident needs as identified by the IDT, including the use of appropriate materials and equipment.
  • Assist in implementing and maintaining an effective orientation program that orients the new employee to the facility, his/her department and its policies and procedures for active treatment.
  • Attend and participate in continuing education programs.
  • Recommend to the Administrator the equipment and supply needs of the department.
  • Assist with planning and monitoring the twenty-four (24) hour schedule for each assigned client.
  • Ensures that appropriate adaptive equipment is present, in good repair, is appropriately used.
  • Provides positive reinforcement to both staff and clients.
  • Informs appropriate staff members of deviations from the daily schedule.
  • Ensures that each resident has a current IPP and active treatment goals monthly.
  • Holds and actively participates in all IDT meetings.
  • Conducts IPP meetings within 30 days of admission, annually, upon significant change, or upon return from hospital or home visit that lasts more than 30 days.
  • Ensures that all assessments are completed in a timely fashion prior to the IPP annually and as needed.
  • Writes monthly notes regarding progress on resident goals as well as family and school contacts and social service needs.
  • Update goals based on review of data as appropriate and in-service staff upon revisions.
  • Notifies appropriate professional staff when goals are met, regression noted, or no progress noted.
  • Ensures that special staffings are held within 3 days following hospital discharges, changes in medical status, or any update occurs which would affect programming.
  • Assure that all clients are well groomed, clean, dressed appropriately in their own clothing, nails clipped, shaved appropriately, hair groomed, etc...
  • Participates in the development, maintenance, and implementation of the facility’s Quality Assurance meetings, etc., as directed.
  • Attends community outings as assigned by the Administrator and/or designee.
  • Assist the Activity Department in calendar planning and special events.
  • Assist the Activity Department in recruiting volunteers and serving as a facility representative when community members come into the facility.
  • Performs other related duties as assigned.
