The Long Term Services and Supports service area within Health and Human Services is seeking two Public Health Nurses in Initial Assessments. The Public Health Nurses will work on an interdisciplinary team of Public Health Nurses, social workers and case management assistants. They will complete Long Term Services and Supports MnCHOICES assessments that are person-centered, and strength and needs based. They will utilize the MnCHOICES tool to determine eligibility, level of care and service needs, and provide access to Medicaid waiver and state programs. These programs serve frail seniors, and individuals with brain injury, mental health, complex medical, developmental, and physical disabilities.

Location and hours:
These positions are hybrid and will be performed both on-site at community locations within Hennepin County and remote as job duties require. Work hours are Monday through Friday, 8 a.m. - 4:30 p.m. Based on current requirements hires must live in or relocate to Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In these positions, you will:
  • Conduct person-centered comprehensive social/health/medical/psych initial or annual assessments.
  • Complete referrals and transfers for service planning and for crisis intervention services.
  • Determine applicants' risk of institutional care and their Level of Care according to state and federal criteria.
  • Analyze assessed data and assist applicants in identifying relevant goals along with specific supports that will enhance a person's ability to meet goals.
  • Coordinate access to Medicaid Waiver programs and Medical Assistance Home Care services.
Need to have:
  • Licensure as a Registered Nurse issued by the State of Minnesota.
  • Certification as a Public Health Nurse by the Minnesota Board of Nursing.
  • A bachelor's degree or higher in nursing from an accredited school of nursing and six months or more of full-time community-based services.
  • A valid driver's license and/or daily access to reliable transportation to travel within Hennepin County work sites.
Nice to have:
Experience:
  • One year or more of home and community-based work experience.
  • Supporting individuals with complex medical needs, serious and persistent mental illness, or developmental delays and the services and networks that serve them.
  • Providing case management.
  • Coordinating access to Medicaid Waiver programs and Medical Assistant Home Care services.
