Our Home Health team provides services to support clients and their caregivers in their homes. Our services range from care for chronic illnesses such as diabetes, cardiac issues, and kidney problems to shorter-term care following an accident or surgery. We also provide care when clients finish a stay in short-term rehabilitation at one of our communities, services from WesleyLife at Home can help keep them moving in the right direction!

As a Physical Therapy AssistantforHome Health you will:
Travel in all weather conditions and to all areas of department service area
Treat clients using a variety of methods
- Work cooperatively with physicians, case managers, and other members of the care team.
- Communicate and coordinate care with Physical Therapist.
- Maintain accurate and timely documentation in the electronic medical record (EMR).
