Supernova Services is continuing to expand and we are looking for a Program Specialist to join our team. We provide supports to individuals with Intellectual Disabilities, Mental Health Diagnoses and/or Autism and this role will be primarily responsible for service delivery, regulatory compliance and team work. This position will be temporarily located in our Washington County location for 6 to 12 months, then permanent location will be Greene County.
Job Description:
· Support the achievement of participants’ personal outcomes.
· Assist Direct Support Professionals in identifying activities to engage clients in.
· Participate in ISP and team meetings.
· Provide general assistance to the participants with implementing their activities of daily living.
· Assist with and provide guidance in the event of a crisis situation.
· Transport participants to medical appointments when scheduled and relay pertinent information to the healthcare professional as needed.
· Provide or assist with medication administration and other medical routines as needed.
· Provide the assistance, training, and support necessary to assist the participant to develop and increase personal competencies and become socially integrated into their communities.
· Follow all support plans for each participant worked with.
· Review ISPs to ensure services are authorized and accurate and services are implemented within the guidelines of the ISP.
· Assist House Managers with coverage within the homes as needed.
· Complete service and progress notes as required by Office of Developmental Programs.
Requirements:
- Degree in Human Services or related field
- Intermediate computer skills
- At least 5 years of experience working in related field
- Driver's license
- Pre-employment drug screening/background check
